Undead Burg: On top of the sewer canal, beside the entrance from Firelink.
Undead Parish: On a corpse in a barrel inside the church, close to Lautrec's cell.
New Londo Ruins: At the end of the walkway along the raised wall, close to the Valley of Drakes entrance.
Valley of Drakes: In a small cave, beside the lone Drake who blocks the narrow path.
Painted World of Ariamis: One on each of the two hanging corpses, cut chains to gain access.
Tomb of Giants: On the ledge at the end of the last slide, right above the first bonfire.
Crystal Cave: Through a narrow passage with invisible floor, after the first Golem.

Bribe from Patches, if you talk to him before the first incident, and don't forgive him after.

Atonement gift from Patches if you choose not to forgive him after the incident in the Tomb of Giants.
Can be taken as a Gift upon character creation.

● Fire Keeper Soul

Locations

Undead Parish. It is on an altar in the Undead Church (2nd section of Undead Parish), guarded by a Berenike Knight.
New Londo Ruins. It can be seen on the right hand when crossing the narrow wooden walkway. Three ghosts will spawn when getting near.
Blighttown. In a wastewater pit, about halfway down from the Valley of Drakes entrance, guarded by fire-dogs and toxic dart shooters.
Duke's Archives. At the bottom of the prison tower, behind the octopus-headed monsters. Need the Archive Tower Giant Cell Key that is acquired later in the level.
After killing Quelaag's Sister. Please note that you won't be able to join the Chaos Servant covenant or use the bonfire in front of her anymore (with the exclusion of being able to warp to, but not from, that location).
After killing the Darkmoon Knightess fire keeper in Anor Londo. If you decide to kill her for her soul, her bonfire will no longer be lit but you may still warp there using the Lordvessel.
If you allow Knight Lautrec kill Anastacia of Astora, then later kill Knight Lautrec in Anor Londo using the Black Eye Orb that was found at Anastacia's dead body (under the Firelink Shrine bonfire.)
This can be used to upgrade your flask but it may also be a viable option to return the soul to the Anastacia's cell under the bonfire.

08.08.2023. Edited text (repair powder) (took me over 1 year, I didnt see comment, my bad) , changed picture.

I feel like repair powder shouldn't say "Repairs broken equiptment". Instead "Refills durability on R. hand weapon" since repair power only effects the R hand weapon as far as I know. Sorry if I come across as rude, it was not intended,

If you use Anastacia's soul to upgrade the Flask, you can kill Quelaag and take the last soul, but you'll only be able to upgrade it to max in NG+

And where is firebomb and BLACKFIREBOMB? Thats not Consumables, but Projectiles (about what u can read in wiki if google "undead hunter charm" - Undead Hunter Charm is a Projectile in Dark Souls 3. (yea DS3 wiki is 1st)).
Here is useless "guide" about projectiles: yea, right here
