Compatible Mid save with some caveats and Conditions
This mod does work mid save if you takes some extra steps and time to ensure everything is assigned where it should be. Before you load your current save, however, TURN ON "PAUSE ON LOAD" IN THE GAMEPLAY OPTIONS.

I also recommend using character editor to make this process easier

enabling this mod mid game can mess up your pawn's current hediffs and lead to some bad situations. For example, one of my pawns with a missing eye and a damaged eye had those injuries replaced with a completely destroyed brain. Enabling the pause on save feature allows you to correct anything before the game has the chance to mess it up

all already generated pawns will be neutered, and their prosthetics might be messed up, sometimes a bionic/prosthetic leg will end up being attached to a toe somehow.

You will need to use character editor OR dev mode to adjust these hediffs, fixing the missing reproductive pieces and adjusting where their bionics are supposed to be. Obviously this will much more tedious the later you are in the game.

Character editor makes it easier by using their health menu. You can set filters on Hediffs from certain mods (like gender works) and then set another filter to only show hediffs for certain parts like reproductive. It'll make this much easier to just click a few times per pawn whereas using devmode can be annoying.

thanks for bothering to write this

so, the body parts getting corrupted - nothing I can do there. this mod adds a new body part (you can probably guess why) and doing that mid-save can will break stuff
reproductives are technically hediffs, yes, but not really. reproductives don't tick or do anything, they're basically arbitrary data points you can slap onto any pawn. this implementation of hidden not-really-hediffs might seem weird but it does knock out two birds with one stone

what you've described is 100% possible but do keep in mind it's probably going to be a nightmare. while you may be able to fix your colonists, that doesn't fix, say, all the world pawns that have been generated. especially with mods like vfe empire which generate tens of world pawns from the start. so while it is technically possible to fix things up, I don't recommend you add the mod midsave
