How to start NCR civil war
im abit confused here, i have done the whole focus tree and the option appears on the decisions tab but its greyed out.

You need to do "create followers" on ncr territory, at least on one

For the US Tree, Once you "Arm Patriots" in a state (as shown in the tutorial event) at least once you can initiate the Civil War.
