This is a setting and not a constant for a reason.

I will consider caravans. As for heroes, it will be addressed in the upcomming v1.2.0 of the mod.

Not going to happen. The more War Exhaustion will depend on dry statistics and the less value judgments will be used in it, the better. In fact, when calculating War Exhaustion, it doesn't even take into account whether you won or lost the battle.
"I would think realistically losing your best, veteran fighters in your army is indeed a bigger form of exhaustion"
I've considered using troop tiers for casualty War Exhaustion calculation, but that would make much more difficult both the perception of the statistics by the players and the actual retrieval of data from the game. I can live with the first one, but the second is a good reason to abandon this idea.
"Another big thing to mention is how unbalanced War Exhaustion can be against factions whom have no fiefs or territory"
Will be addressed in v1.2.0. Fiefless factions will be destroyed when loosing their last ongoing war due to War Exhaustion. And they will receive a considerable amount of War Exhaustion for each day being without fiefs.
Overall, v1.2.0 is mainly focused on War Exhaustion, there are a lot of changes that I hope will make this mechanic much better and clearer.
