Check that your mod is updated to current version on your server and use the standard CBA settings in a fresh mission file.

My server says on startup:

Thank you for this bug report. I have released a patch that addresses this concern.
I had commented this line out for some reason in the latest update. Now the code checks that the hull, main rotor, or engine are above critical damage thresholds to initiate the script.
As always if there are further suggestions or reports please send them my way.

This "Survivable Wrecks" main fiture is controlling and capping damage crew recieved on vehicle destruction (fireball state), so crew get fallen out of destroyed vehicle, taking CBA Desired damage cap, and no one ever die if cap is low.
You can crash plane directly in ground and still survive (e.g. got flipped due crash landing of helicopter because Arma' moments, and be alive), so they both working but bit different ways
