Out of curiosity -- does the problem happen consistently, or only on certain maps? Also, have you tried from a fresh, non-previously saved game state?
The best way I would imagine to do that is to decompile this model, decompile the corrected/improved hitboxes Bullsquid model, use the bounding box information from that mod (I think it's in the .qc file, IIRC) -- recompile.
Alternatively, you could also copy the non-eye materials to be used with the default model (the UV maps are identical to the base game's model) -- The teeth, eyelids, inner eyes, won't be present, though.

You'll have to do some texture editing since the 2006 model uses eyes on the same texture sheet as the rest of the skin (which I repurposed for the eyelid geometry).
how to get the red color?
You might have better success launching a new game from the same chapter or map.
(Also, be sure you aren't using any other mods that would overwrite the bullsquid model)
@Skyguy -- same might apply to you, if you started a game/made a save with the mod enabled you should try a fresh game. The Black Mesa workshop has historically had weird quirks with cached assets.
