1. Did you set it up before the campaign started?
I made this mod impossible to change options within the campaign.
2. Did you use the latest version of MCT mod?
Old version MCT mode name is "!! MCT BROKEN, FIXED VERSION IS IN DESCRIPTION !!"
If none of the above two cases is the case, I don't really know why.

Can you check the 'Always Spawn Camp' option and start a new campaign? If so, you can spawn a camp after first battle and you'll know right away if the mod is working. If you test it with Mannfred von Carstein, you can check it as an auto battle in turn 1.
First of all, It is a bug that the dilemma does not appear. It happens time to time, but I haven't found a reason yet. Sometimes I heard that vanilla ogre camps also have a similar bug, so I think it might have something to do with it.
The invisibility of the MCT option is intended. Since the option is set at the start of the campaign, I made it can't be changed in-campaign. The MCT option can only be set before starting the game campaign.
Lastly, the order of the mod is irrelevant.
