why you don't provide an in game icon for the map? just take the image from above resize it to 128x128 px and save it as png with same name as the map and place it in the maps folder.

Garry's Mod use resources(models, textures) from other Source games, if you not have CSS or HL2 you can download textures from http://gamebanana.com/

probably you not properly configured hammer, i used a hammer from CSS \Steam\SteamApps\common\Counter-Strike Source\bin\hammer.exe he has no problem with settings)
