The facials are just parts of the blink/talk bodygroups trimmed and placed inside the head. When they're used, they come out of the head and cover the regular textures behind them.
The eyes were just traced with the cut tool in Blender.
That's why Low Model/Texture settings or being far away disables facials; the talk/blink textures would begin to Z-Fight over the regular textures from a distance. If you're still confused, I recommend decompiling the model and see how it was done.
