I might just have to retire this mod if that's the case.

How exactly are you patching it? This mod doesn't remove the ship health bars, per se, it makes them transparent and therefore invisible. If that mod is loading after mine and overwriting it, whatever graphic it's using will appear instead of the clear one I have in this mod.
Unfortunately https://psteamcommunity.yuanyoumao.com/sharedfiles/filedetails/?id=2409209888 doesn't play too well with this mod. The hull & armour bars remain on ships & mining stations despite the last couple hours of me trying to patch it with Irony. If you've got any suggestions it'd be much appreciated.
I'm afraid I don't have the coding knowledge to change how things are displayed in the UI. All I can do is change the image that is displayed, which is a simple art adjustment I can do in GIMP. You'll have to find a more knowledgeable programmer to make a mod that selectively turns names on or off based on conditions like being colonized or not.
1. Hide system name which do not have an colonized planet in galaxy map, sinc they are not important and their name clutter the map. You can always hit Alt to see those name anyway.
2. Simplify the ship icon by removing the ship from those ship icon. For example, navy icon show only star.

Do you know for certain if this mod has this problem when run alone? Or is it caused by an interaction with another mod?
All I know for certain is that anything which changes the ribbon all the icons are on, will mess this mod up. You will likely have to make a version of that ribbon yourself which looks like mine but is compatible with whatever other mod is changing it.
I'm trying to figure out if there's values I can tweak somewhere to just fix the particular ones that aren't correct? Or is it more complicated than that?
Just disable "Smaller Resource Icons 3.0.x" and Foxt's mod works perfectly well.

I like the colors the way they are, and while it's possible to add variations for them I honestly do not want to commit myself to upkeeping several versions of this mod that are palette swaps. This one's hard enough for me to keep up to date and functional. You are welcome to try your hand at customizing it, though, and if you do I'll link your work in the mod's description.
Is there anyway you could have the planet names grey instead of white? I find the white text quite immersion breaking as the white really stands out. The un-surveyed planets have grey text names and it looks so much better. Changing any white icon or text to grey would be preferable.
