It has 2 problems that cause it, the icon is called "icon.png", it should be called "icons.png", the second problem is that it is not the right size, it measures 16x16 and it should measure 64x16.
You have to make a 64x16 file and put the bag icon on the right side almost, editing the created png, then save it as "Icons.png", problem solved.

You put the PNG in the address I put above, you have to unpack the .pak file of the mod first, obviously, search how to do it in google =)
