A bug I've encountered twice now is Old Skull appearing on the empty pedestal in boss challenge rooms after the original item has been picked up. Bumping into it didn't pick it up while the challenge was ongoing, but it did after the bosses were beaten.

i just did 12 boss rushes in a row and didn't get it once so I don't know what's going on with your game

you know what? considering everything in golden chests being head or head related, it could fit there. cause y'know. skull.

its not actually in any item pools, it just has a chance to replace a boss item if the defeated boss was one of those 4 and you don't already have it, all through making and testing it I never actually got one of them to spawn naturally (at least not before getting to really late levels where bosses just spawn all over the place) and there were a bunch of times I defeated the boss but didn't get the item, so I feel like it's rare enough

Feel like its kinda OP for the boss item pools. It feels better fitted for more uncommon pools, like chests or devil deals. Otherwise its pretty neat.

Good idea, we need more bone items. I think that NIcallis should include a bone transformation called osteoporosis, that grants damage and three bonehearts, but it makes them break if they´re empty

You should be able to get it as the forgotten with a custom costume where it sits on top of his skull
