or you are using a mod that somehow removes it.
you can see the differences in the FCS
it's what gave me the most issues because the numbers didn't add up
best you can do is place two mats next to each other like shown in the picture.
you may edit this mod to make it go higher for yourself but you may not publish it.
It is a known issue by the Devs
if you want them to only go up so much, then you need to subscribe to that one.

only way to lose limbs is in combat with NPCs or use of the save editing.

No matter which "sparring mats" you use, 1, 2 or level 3, the training does not go beyond level 14. Maybe only the level 1 mat is set, it's hard to see in the UI.
