-Heavy improvements to performance

I finally realized that this addon was causing heavy performance issues, even for my high-end PC. The code has been formatted in order to maximize performance as much as I can think of. Weapon icons may still cause issues due to the fact that they use a performance-impacting method for finding the proper entities.

However everything else should now have minimal effect on performance.

I regret not figuring this out earlier.

-Fixed Combat Stims causing errors due to a missing distance variable

-Changed code a little, not sure if this will have any effect on performance

-Ammo icons now show up on HL2 shotgun ammo
