It should work by simply subscribing.
In case it doesn't, after the file is downloaded after subscribing, locate Steam\steamapps\workshop\content\362890\2781238348 and copy the 2012MP5HD+.vpk into Steam\steamapps\common\Black Mesa\bms\custom and you're good to go. In case there is no custom folder, create one. It is recommended to primarily use the /custom method for mod installations as it's more fail-proof and easier to organize.
