Strange thing is I didn't need to turn off all checks.
Perhaps the pocket actives + mod might be involved?
Side note, would be nice being able to do the same feature for Q items, but I'm not sure how difficult that would be.
In regards to activation factors, there are only a couple of checks being done, and none of them should prevent you from doing what you were trying to do (even with default settings). I've added a new setting to ignore all checks anyway though, so feel free to try using it. Not sure why you're having issues with all characters though, it seems to be working fine for me, so perhaps try checking the console or making sure you don't have any conflicting mods installed.
If there's any other factor besides the mod just not activating, could you make sure any deactivation factor (like custom characters or items) can be toggled off?
I got guppy's head in the red redemption challenge and it didn't work there either, even though it's just a weird tainted Jacob.
