Maybe a new update of Gmod broke it. I am not active in the Gmod community anymore.

Mr. Bigglesworth, check your playermodel. Sometimes some models aren't made correctly so there is geometry literally stretching across the map. If this map has any trigger_hurt around, then that's what's killing you.
