How does radiation work? And how long do you have to be in the rad zone before you start receiving damage? Also, does it have delayed effects?

I know you get the "drunk" and "sick" effects when you go to the fallout zone without a suit, but I figured you can walk like that for quite a time and be completely fine afterwards, despite the fact that you'd realistically get terminally ill and die shortly after.
