I think what they mean is that it hits the player, rather than the city, because it hits you even if you're in the middle of nowhere, which is quite weird.
Hm. I watched a couple streams of the Day One mod and in both cases they were outside the towns and the nuke dropping dead center onto them.
One IIRC was between the two farm houses NW of Rosewood, instantly killing all the NPCs they were struggling with, leaving the player with burns.
The other was driving on the highway I think NW of Rosewood / east of Echo Creek on the big curve or something with forest on both sides.
I've yet to see someone actually surviving the Week One mod to reach this point, but in the Day One mod it's definitely targeting the player, just like all the other bombing & gassing happening only around the player.
Im on day 21 now :D the event is totally redone and much more atmospheric now, but as far i explored, i spawned in West Point and only that and a small surroundign area got ♥♥♥♥♥♥ by nukes + contamination, all the rest is vanilla zomboid plus occasional raiders (not as annoyng as week one which is a plus)
I survived my first playthru and survive more then %70 of the time until nuke drop. Its not that hard if you act like a real human and dont go looting your neighbor day one. Run from fights like a normal person. Only fighting back when having to cause its just to dangerous. Play it like real life and not a game you will make it to nuke drop almost always but what come after will be way harder.
