Works for me, can't fix an issue that I can't reproduce, sorry. The obscure issues that seem rare or hard to replicate are probably ping related but I'm just speculating.
If a new card has been taken out then you didn't "Remove the Insider from the game". The rulebook has a section dedicated to this sentence:
As you can read, if there's no Story cards on the table, then it's impossible to draw them. So you didn't actually proceed the intended way. Delete them or put them in a bag, or the box, don't let them be on the table.
Anyway it gave no issues to the gameplay whatsoever but i will let you know if i can do it again
