Can someone confirm whether or not girder blocks will block thruster propulsion if they are placed on top of the exhaust part of the thruster? I see POI designs like this but want to know if it hinders speed? Thanks

Doesn't matter whether it's vanilla or a custom scenario, blocks don't prevent thrusters from working. You could bury them inside if you really wanted.

oh nice I had read somewhere there needed to be an air gap of one space at least for regular blocks but wasnt sure about other ones. Thanks for the info :)

There was a (vanilla) version a few years ago that had that, and I thought it was a good idea. But there were problems with it, maybe just the load on the player's computer. So they turned it off again.

I still play like it was on, though, except for the vent cover blocks.
