I can't figure out how to use the firebomb on the gate

No worries, I'm glad you found it in the end :)

There are a couple of other places where ALT does not illuminate an active element. That makes them easy to overlook. I'm used to checking just theglowing items.

I know about one not lighting up in the wizard tower and some in the tower library that may be a bit hard to see, but those are in such small rooms that I found them fairly easy to find with just mouseover in testing, so I kept them that way. But as far as I know those are the only ones. I'm not sure, but some others might be the game itself bugging some.

There's a dialog on the table in front of where you teleported up (there's one near the entrance of each floor which will tell you a bit about the room). After activating that there's a dialog on the wall behind the teleport platform which will start a quest on how to activate the teleporter. That should tell you what to do, though, depending on rolls, you might have look around a bit to find everything.
