Yeah anything that modifies the cities can potentially cause issues, particularly with a mod list like this. You'd need to have probably multiple patches to ensure the cities aren't overlapping and such, as well as the various NPC additions that a city mod might entail. Kind of what you said with the NPCs and trying to figure out their jobs and *how* to route their path, if that makes sense.
I do have a few performance related mods on the list which helps most people, but various things like disabling "Distant Towns" in the graphics menu can help (as well as help prevent random CTDs, which is seemingly random based on computer specs since it works fine for me).
The game is very old, and somewhat unoptimized (particularly with mods) due to the engine and how it works, so even with modern hardware you can't really bump up the View Distance even on a 3080+ GPU lol, especially with mods.
