A formal way of reporting a crash.
This exists to help me diagnose crashes.

- What caused the crash.
- What addon was involved.(if possible the link)
- What branch of gmod are you on. (If you don't know just say normal).
- If it happened in multiplayer or singleplayer.
- What map where you on?

- Roughly how long you were playing for.
- Roughly how many times you had used the crucifix.
- What npc did you use it on.
- What your crucifix settings were.
- Version info (Options->Crucifix->Help and Info)
- If an error occurred before/after and what error.
- Was the game lagging before the crash?
- Any other information that may help.

(Structure doesn't really matter.)
I tried to crucifix rush in construct and I was using DrG doors nextbots.
Also, I was using normal gmod in single player.


If a report is fixed I will delete it(you can re-report if it is still occuring). If a report gives little evidence or is a joke it will be deleted. (If people are having similar issues please don't reply because it will hide other crashes)

I'm making these requirements so I have the best chance at fixing a crash.
