I seem to be able to voluntary trigger/reproduce that with the following 'trick':
- Have crew with personalized outfit
- Have a NPC (villager type), non crew, underwater -she's a mermaid okay, she's fine- on the ship. Yes, I have an indoor aquarium.
- Go in the aquarium, "E" the mermaid... which does nothing, since not talking underwater
From then on, when trying to interact with any crew on ship, it does nothing. But as soon as you warp out and warp back in, all their outfits will reset in the exact same way as it can also randomly happen that I reported before.
