Gather Companions means that all companions will appear in the first tavern. Fog of War means that the settlements on the map will be hidden until the player is close enough to see them.

You can always just use cheatmenu to give you some equipment when you start, ctrl + ` then type in cheatmenu

Well even though this map is getting more trash per update (love you cozur but spearmen do 0 damage) the tactics i'd use is 'gather companions' and then borrow martyn snow's arming sword as your rusty ♥♥♥♥ will do 0 damage. then explore all ruins and it gives you some good ♥♥♥♥. especially vultures roost
