The above thought makes me think its a map issue, similar to the one we tried to fix. Does this issue still pop up on gm_construct or flatgrass? If so, then it may narrow it down to an addon conflict. Otherwise, it might be a issue with maps; from there, we can continue that discussion
Ragdolls are entities. You have to uncheck "Disable entity selection" (or similar depending on your translation) so that you can properly select anything. Also, does this error pop up consistently with just ragdolls? Test with regular props to see if that's the case
Finally, on toolgun selection, how is it inconsistent?
