How do we add swords and stuff to a character? (not solved)

My guys always teleport under the map. The only way of having characters hold anything is to make the weapons have skinned mesh renderers, but whenever I do that, it makes the teleporting under the map happen. It seems to only happen on this specific animation type (while having a skinned mesh sword).

Oh is this what causes some peoples mods to be invisible on the map and instantly die and lose, like a few in the video i did showing some mod issues?
