The problem is that there's no easy way to detect if damage is an actual "headshot" that's generic enough to work on all guns / weapons.
You can try enabling criticals with "hitmarkers_criticals", which'll display a different coloured hitmarker if you deal a large amount of damage at once. For most weapons, this should be good enough for headshot detection.

can you add hitmarker volume though?
