No clip the effect, then use the physics gun to move the effect on the rag doll of where you want it to be

Go to the spawn list, addons folder and it should be under the same name as the addon

You noclip rag dolls and objects by using the no clip tool and selecting the rag doll and the object you can want to no clip. or right clicking them by holding down C and click on no clip

You paint them in the TF2 hat painter tool, no clip them to tf2 themed rag dolls like say Pyro for example. Put it on a spot of the pyro you wish to put it and boom, pyro now has a shot wound for gmod artworks.
