I've published this privately so I can test it in a live environment. So far the penalties are working and you're restricted to town respawn only. The portals are closing on death as well (PORTALCLOSER under MENUS is firing).
All my leveling lag went away. I have no clue why I'm the only one noticing it.

In an previous mod of mine i had tried to change all the boss encounters so that when you die, they regenerate.. Actually, it was done by disallowing resurrection in place, and putting a sphere around the boss area - whenever you exit that sphere, the boss regenerates. But it was painful because i had to modify every single boss encounter (i stopped at act 1 lol) to make them "closed" (as in, you can't get out once you're in until you killed the boss) and add the sphere stuff.
I'd be really curious to see how you managed to heal all monsters in the vicinity when you die.
You and doudley release really interesting mods these last days, that's cool to see ^^
Coupled with your crit mod, I've found it really fun.
This feature is exactly what ToughCore deserves, I think it is now complete!
All enemies within 24m will be fully healed when you die (including bosses)

You have to make a serious logicking for that :P
Try to look for how the game would know if the player is on MP or not...could be in a form of stat, if you have not found any, you can create your own stat that can tell your logic group instead.
I thought the portal get closed on resurrection menu's opening....the health check is not needed anymore, I think.
