It doesn't appear in the logo folder, also I don't find the files anywhere on my SSD. I'm subscribed to it, but there are no files. I can't understand this. Not working for me.

Well, this is bad timing because Valve decided to patch the game, breaking some addon on the way, including mine : "exec [PW]medcab_FadeOutSpray;" can't work anymore.
Last solution I can give you to select a spray is to input one of the following in console :
cl_logofile "materials/vgui/logos/custom/medical cabinet.vtf"
cl_logofile "materials/vgui/logos/custom/medical cabinet - 2 frames.vtf"
cl_logofile "materials/vgui/logos/custom/medical cabinet - 3 frames.vtf"
cl_logofile "materials/vgui/logos/custom/medical cabinet graffitis.vtf"

Before, be sure to have subscribed back and that the addon is enabled (by checking it in L4D2 addon menu).
If this still don't work, I'm deeply sorry I couldn't help you and I can't think of anything that could possibly fix it.

tbh i just uninstalled it after i tried both methods there were to using it cause i got tired of trying to get it to work. if you have any alternative fixes let me know

Check if you've enabled the addon in L4D2 addon menu, because it looks like you don't have it at all.

I tried using the console, and what came up was
exec: couldn't exec [PW]medcab-graffitis_FadeOutSpray. I tried all 4 different things it said to type into console, none worked

Please try the second installation option by enable the game console https://www.google.fr/search?q=left+4+dead+2+enable+console
and typing "exec [PW]medcab_FadeOutSpray".

I know, that's what I'm clicking on, and what I said is what comes up for me

It's the "custom spray" option from the bottom of the list that appears when you click on "Spraypaint Image" from the multiplayer menu.

When I go to select custom spray, all that comes up is a folder list. There's no straight up sprays to choose from up front. I have to go searching through folders to put in a custom spray

It's because you're doing it wrong, you don't gave to go in any folder.
Use the "Installation" section as reference.

After I go under the logos folder, theres nothing that says custom.

The addon file contain the following directories and texture "materials/vgui/logos/custom/spray.vtf" among others things.

The game will do all the work for you, just follow the "Installation" part and you won't need to go in any directory.

What's the directory to look in? If the textures aren't extracted, how will they be selectable at the Import option?
