You're probably missing a namelist for the new ship added in federations
When I made this name list I decided that it would be varied and that's something I'm sticking to.

I've been wrestling with computer issues the last couple days so I haven't had time to take a look at it, but unless there were undocumented changes to how namelists work it *should* work perfectly fine.
However, if you've already encountered issues, please let me know about them and I'll try to expedite getting a fix out for them!
- Removed one erroneous battleship name
- Removed duplicate lists showing up in empire creation
I'm also still trying to figure out a naming scheme for fleets. While Inspector DuoDex is probably on the right track I cannot really get behind any of the options. There just doesn't seem to be a good way to get in some sufficiently British sounding/semi-historical names in a fashion that doesn't make it ridiculous due to the random selection that then smoothly segues into generic names as needed. Further deliberations to follow!
Force A, B, C, D, etc.
Red Squadron, Blue Squadron, White Squadron (more colors if you want?)
1st Squadron, 2nd Squadron, 3rd Squadron, etc.
1st Flotilla, 2nd Flotilla, 3rd Flotilla, etc.
- Compatibility for 2.0/Cherryh and Apocalypse
- Major overhaul:
-- added one name for colossi: Incomparable (no other seemed appropriate)
-- added sixteen names for titans: Agincourt, Anson, Barham, Beatty, Duke of York, Hood, Howe, Jellicoe, King George V, Malaya, Prince of Wales, Queen Elizabeth, Rodney, Valiant, Vanguard, Warspite (that is all fast battleships built/planned, plus Hood as a quasi-fast battleship)
-- names for military stations consolidated for defence platforms and ion cannons (in game these will mostly be used by the designs, built platforms/cannons get Roman numerals appended)
-- number of battalions for each historic name has been bumped up, possibly excessively so
-- all army types will now be named in the scheme 7 RIFLES, 11 RIFLES, etc. while defence and assault armies have a chance to take names from current and historic formations
-- major overhault for leader name generation
--- split names into ten categories with varying weights
--- names of British origin should be a lot more common now while names with different origin for first and second names should be less common
--- name categories are as follows:
---- general British names (plus one or two outliers), weight 80
---- Arabic names, weight 15
---- Indian names, weight 20

---- Irish names, weight 10
---- Scottish names, weight 5
---- Welsh names, weight 5
---- mixed names (all names from the categories above in any combination), weight 20
---- knighted British names (first category, prefixed with Sir/Dame), weight 10

I'll update the namelist as soon as possible, although I fear I won't be able to dive into it until Sunday. Maybe Friday, if things pan out favourably.
Until then this is most likely not going to be compatible. I haven't checked yet, but I suspect that with starbases and whatnot namelists might've changed in their construction. Either way, investigation and update, as well as a possible expansion in scope depending on what new things can be named, are in the works.
Until then, try out some of the new vanilla name lists for humans and blow up a planet or five!
- Regnal surnames have been removed as suggested by Adm. Yanxa, meaning that rulers using them will now only show up with their first name. Worry not, however, the surnames Windsor and Wales still can appear on regular leaders, representing non-ruling members of the royal family.
I'll try to see what happens if I leave the regnal surname empty. Or whether full names, which definitely work for normal leaders, are an option for monarchies, too. If either works I'll change it around to that and upload an update.
Windsor and Wales are in the normal surname list for leaders already anyways, so non-ruling members of the royal family could pop up that way.
- Fixed compatibility for 1.3 - Heinlein (hopefully).
- Added a number of generic planet names referring to people important to their history (e.g.: Atkinson's World).
- Added a workaround to enable random names for Assault Armies, Defence Armies, and Fleets. Enough names have been provided to hopefully prevent wraparound even in atypical games with hundreds, if not thousands of fleets and armies of those types.
- Added a variation of the name list that comes with the prefixes included in the ship names, this way different types of ships use different prefixes. All military vessels and all those from other categories that got a generic name: HMS (His/Her Majesty's Ship); all construction vessels, colony ships, and (in theory) transports: RFA (Royal Fleet Auxiliary); all research vessels: RRS (Royal Research Ship).
As for including the name list in other works, I'd prefer the use of collections or required mods instead, since it allows me to more easily maintain the name list from one end instead of having to also send out updates to other modders who're using the name list (or parts of it) for their project. You're not the first to ask though, I'll mull it over and get back to you!
On another question: Would you allow me to incorporate parts of your list into my own namelists? I will of course credit you and link this mod. I intend to make a human namelist collection.
