To set and activate it go into the PIE menu while holding it.

Thats a separate mod module called Incredidoor. It should come bundled with Map Pack 3 though if you want to install only it directly, heres the link for it;

I wish i could, that mod has some amazing mapping assets. The dropbox link is gone and i'm not sure where else I would find it, the OG's on the forums might have a copy or something lying around. Ask around there.
