I have a custom vpk that enables them for the healthbar portraits too, which is what I believe you are looking for. Here you go! http://www.volcom-mapping.com/hlmaps/z_l4d1_lobby_portraits.zip

It already includes these lobby icons so you wont need to subscribe to this anymore.
