In your Mods-directory (%AppData%/Space Engineers/Mods) you must create the folder with 'YourModName'. Inside that folder you make your Data and Texture folder. The third thing to add there is the image you want to be your mod-thumbnail. As far as I know it has to be a .png image. It should have a reasonable size, so not to small, not to big. This image must be named thumb.png. So rename it to that.
Now you do have the folders 'Data and 'Textures'and the image 'thumb.png' in your directory %appdata%/Space Engineers/Mods/Yourmodname.
