If other background mods don't seem to work for you also, a fix I've noticed is changing your video settings anyway possible (ie. LOW>High, Vice Versa) . As long as it reloads the game the background will load. Just a heads up.

Subscribe to the item, start/exit the game, then navigate to

Steam > steamapps > workshop > content > 222880 > 402870046

move the .apk file to

Steam > steamapps > common > insurgency2 > insurgency > custom

Assuming that you have no other mods that replace the same files, it should work on startup
