Everything is fine when I test it myself, so might be because of other mods you installed?
Would be convenient if you can provide details like mods you installed and game version :)

If Tainted Lost picks up a Quality 4 item count it as a hit for the counter ( hardcore mode )

@Pokechu08 Strange, I just tested it for REPENTOGON and it works fine, maybe you could try unsub and re-sub the mod to see if it works? Btw make sure you have latest version of REPENTOGON.

the hud overlaps with the planetarium chance (cuz Repentagon has it built in)

I think I'm gonna use yours because I like the funny Isaac head

And I don't think I need the perfection floor counter thing
