what shadrar is referring to is the fact that the negative fame payout together with being stuck as slave makes victory impossible - what is supposed to increase fame payouts is instead multiplying the negative fame - making you lose more than you can possibly gain
unless I'm missing something. can you make your fame go into the negative with gear?

Not really! It should be harder but there's a possibility you win. The sick trait will never bring your health lower then 1hp.

I'm guessing it's supposed to be impossible to win but it's not
