Renamed the "Truffle_Default.png" file to "NPC_160.png" like you suggested. Still didn't work even when copying over to main image folder.
I also found the same issue in a separate mod: "Short-Haired Royalty" (3029234729) Where the pack replaces nothing even when enabled and put at the top of the priority list.
I hate when a problem hasn't been reported on and no-one knows what to do. It's very irritating. And people will continue to not report the problem, because it isn't a big deal apparently. But it really is.
This has to be a problem with how Tmodloader loads in resource packs, because as I said, default Terraria is just fine. I disabled all mods, and put the resource pack at the top. Just to see it not work yet again.
And just fyi, I put this resource pack at the very top above all mods and it still didn't work. I disabled all my workshop mods for Tmodloader, and even then, it still didn't work.

But you can still give it a try with your own take at respriting them! I dont remember seeing any NPC replacement with Marisa and Remilia anyway
