If you insist on keeping the model I sugest adding reddish-brown streaks and tint to the handle to turn it into an old, weathered, bone. Currently it's a tad too bright IG.
I do like the idea of this weapon, but I also predict that it would be a pretty big noob trap for the Spies out there who can't backstab, and would rather lean on the damage boost to carry them through. For this I would suggest an initial speed reduction to punish reckless use.
To the pros who can trick-stab consistently, the cooldown for backstabs would discourage them from using it, as it would hinder their otherwise short kill time, so they're better off using the Big Earner or the Kunai still.
Lastly, despite the aforementioned issues, this knife would really piss off Razorback users, and for that its a solid 10/10.
The innate damage boost from the knife would not be so much an issue at first, but if a good Spy can stay alive long enough to build up their speed, there will be many complaints from the enemy Snipers and Scouts complaining about him chasing them down and butter-knifing them to death (which will be very funny, because I hate these classes).

As well the stats I suggested are *not* how they'd make it into the game, if Valve adds this knife they'll do the stats and balancing. Just the idea I had I think would be pretty fun.
