No you do not need the old version of the patch - but HTLS marks your save with the mod so you can only use one save with one mod.

Does it require the Old Lash Patch or would it be wise to unsubscribe the old version and subscribe only the new one?

Its not copy and pasted straight out of the llm. Its had a 120hour play through plus over a month of manual cleansing. You can see from the commit history if you dont believe me. @nascentes
