Yeah the devs never really thought about the volume when making this game it seems, I have volume 0.02 in my autoexec and config file.

We need to start a series of decreasing noise mods and making the game more chill, I have to play the game at 0.008 volume (yeah actually) otherwise all the noise really destroys my ears
