It starts with a color, and the more it reaches Medic's shoes, the less visible the paint gets. Or ultimately, only making the chest area be affected by paint, even if it uses the same colors by default.
That one Soldier skull cape item does this. From the front and back, it's the same color, but when painting, only the front side is painted.
Chest part is ok, but inside part of a robe has massive paint region which can be seen from far distance. Visit Loadout.tf and paint red medic to a Color Similar to Slate in order to understand what I mean.
It's on an area you'd only see when facing Medic, and even then, he's almost always going to be having his massive, team coloured hands over the front when holding any of his weapons.
