It would certainly be possible, however it wouldn't sound right since the usual way would be to replace the pill opening or pill use sound file, but they need to have set durations, so you can't replace a 0.5 second audio file with a 5 second one, for example.

Ah, what if the perk bottles were set? Like if there was just a mod to have juggernog replace the pills, so then it'd play juggernogs jingle. Is that possible as that wouldn't be randomized?

Not possible to have it randomized, since there is currently no way to implement random audio files.
