Games with Discord's Rich Presence support simply report what is going on in the game by communicating with Discord, the downloadable program, and having Discord decide what to do with it (usually making it show up in the activity tab on your profile).
This mod will make the game report the stuff mentioned in the description.
If Discord is up and running, then it will pick up on that a game is communicating with it. If Discord Rich Presence features are enabled and it is displaying your activity as your status, then the game will use the information given and make it show up.
You obviously need Discord for this to work. It doesn't message you. You don't need to join any server.
