I'm not sure if it will help the deformation issue but if it dosen't correct itself the second thing I would try is by going back to the model and try increasing the weight of her face so that it becomes static with no facial anims may resolve the problem. It looks like it is caused by her chin trying to snap into place with the default skeleton, that causing her jaw to become pushed inwards making the strange derpiness of the face.
It worth a try if it helps.
I'm not sure if it will work but try setting her stats or skeleton to the one the citizen have. Citizen models tend to have random faces when spawned eg: they look different from each other, where as alyx doesn't have random meshes and her skeleton is not a universal skeleton as the citizen ones are, it might be the cause for her face to drop out of shape. modders tend to use citizen properties for almost all NPC and player models. That's my only guess anyway I hope it helps.
