also you have a lot of materials (sprites) missing, probably you haven't packed it into final addon

to have an icon in main menu the file in "thumb" folder should be called same as the bsp file (gm_okinawa.png), not the icon.png

It's a model, so putting $mostlyopaque at the top of .qc will fix that. Alternatively, you could try these .vmt parameters for smooth translucency with proper sorting:
"$alphatest" 1
"$allowalphatocoverage" 1
