Was this the 7th/last player or one of the first six players? Bot or real player?
Did this player spawn there right away at the start? How did they get there?
As many details as possible would help figure out what's causing the issue.
If you tried the game again, did the same thing happen?
Update should address some issues and make it more clear when the auto selection process triggers (via the new descending laser scanner).
Player couldn't decide in time, and the game has an automatic switch selection when a player cannot decide after ~10 seconds. The auto selection picks the nearest switch to the player, which was green, but the player also hit the yellow switch at the exact same time. When the auto selection picks a switch that was already triggered (green switch), it will kill the player. Since 2 switches were triggered, it triggered the queue to move twice. That is how green and pink were both on stage at same time.
I will work on an update to see what I can do!
Purple player at start:
When the minigame starts, it sets player speed to zero. I believe the game sometimes accepts input during the action events of the minigame triggers. Purple was probably moving as the game started/loaded, so it preserved his momentum and caused him to slide off. This is a bug I noticed in testing but only on stage, not on floor. (I test with bots which don't move when on the floor).

Sounds like possibly a network or sync issue if this was an online match.
Unfortunately, I don't have an idea for how to reset the switches when there is 1 switch left. The current reset logic relies on the rigged switch being activated.
