clearing some confused people up the community is very decisive on if its a 50/50 a 20/80 or just luck based if you kill little Steven first, most people believe its a 20/80 and we don't really have enough information or if we do I didn't find it.

If you have There's Options, this mod seems to separate the 2 items so you can take both (OptionsPickupIndex property). Using Morph instead of remove/spawn should fix it.
